FAQ: SUPPORT COORDINATOR

  •    Support Coordinators support participants to understand the planning process and manage their NDIS funds.

A Support Coordinator must work with  a participant to:

  1. Monitor their budget
  2.  Ensure they have enough funding for continuation  of supports for the duration of their plan
  3. Confirm they are receiving enough funding to work  towards their plan goals

A Support Coordinator will work with the  Participant to monitor their plan budget by:

  • Calculating the amount of funding available under each  category and what this equates to in hours of support
  •  Implementing the plan using the hours that are  allocated to Support ordination

Working with the Participant to identify and discuss:

  •  The range of available NDIS-funded
  •  Informal supports
  •  Community supports
  •  Mainstream supports (including other  government and community services)
  •  Exploring how a Participant can exercise choice and control when choosing their providers.
  • Connecting with NDIS providers and other supports and services that they require.

Play a key role in resolving issues and points of crisis.

The impact of these events may be significant and  can be referred to as “crisis” events such as becoming  homeless, going to a hospital, or experiencing a  significant loss of a carer or family member.

 A Support Coordinator can work with the Participant to;

  • Create a plan detailing what they can do when  these situations occur .
  •  Put additional supports in place to help them  work through these crises

The NDIA also requires Support Coordinators to report on the outcomes of a Participant’s plan at various stages of implementation.

  •  An Initial Plan Implementation Report  This report is completed 8 weeks after a  Participant’s plan begins or the Support  Coordinator accepts the request for service.
  •  A Mid-Term Implementation Report  The report is most commonly requested if  the Participant is accessing the NDIS Complex  Planning Pathway supports. How often these  reports must be completed will be provided by  the request for service and the Complex Planner.
  • A Plan Reassessment and Evaluation Report

This report should be completed before the  Participant’s next NDIS review

plan review. The Participant and their Support Coordinator reflect on how the

             plan has been implemented.
